NEW PATIENTS

Your initial appointment may take up to 60 minutes and will include at minimum a full history of your presenting complaints and general health status, as well as a physical examination of your neuromusculoskeletal system. In addition, if you have relevant imaging ( Xrays, CAT scans, MRIs) of your primary issue, we will arrange to obtain them as well as other prior records to review at your initial appointment.

To schedule an initial new patient appointment:

• Call (507) 263-2393 during our regular business hours and talk directly to a staff member, or

• Use the online scheduling link on the website , or

• Fill out the online request form below and a staff member will get back to you within one business day.

• Complete the New Patient Intake Forms and email it to us at frontdesk@bwclinic.com. (See instructions.)

• If filing to insurance, please email us a photo of the front and back of your insurance card.

Intake Form Instructions: After completing your forms you need to download them and save them to your device. (If you skip this step we will receive blank forms.) Then email your completed forms to us at frontdesk@bwclinic.com. Please don’t complete them and then print them off as it will not show all of the information you entered. Insurance

We are participating providers with several health plans listed below, and we routinely work with patients who don’t have chiropractic coverage or choose not to file through insurance.
