NEW PATIENTS

Your initial appointment may take up to 60 minutes and will include at minimum a full history of your presenting complaints and general health status, as well as a physical examination of your neuromusculoskeletal system.

To schedule an initial new patient appointment:

  • Call (507) 263-2393 during our regular business hours and talk directly to a staff member.

  • Complete the New Patient Intake Forms and email it to us at frontdesk@bwclinic.com. (See instructions.)

  • Fill out the online request form and a staff member will get back to you within one business day.

We work with several Insurance Providers and we also offer a Direct Discount Plan to patients who don’t have coverage or choose not to file through insurance. Click the links below for more details.

Intake Form Instructions: After completing your forms you need to download them and save them to your device. (If you skip this step we will receive blank forms.) Then email your completed forms to us at frontdesk@bwclinic.com. Please don’t complete them and then print them off as it will not show all of the information you entered.
